Life Support Supervisor

Mobilization:15th December 2012
Working Shift:2/3 weeks

In order to proceed with the work permit application we need the following documentation:
1)      Curriculum Vitae *
2)      Professional Certificate – IMCA approved
3)      BOSIET – OPITO approved
4)      International Diving Medical Certificate**
5)      H2S Certificate***
6)      Yellow fever vaccination
7)      Passport****
8)      Seaman Book
9)     Photo – jpeg

*CV – up-to-date, including home address, telephone number, e-mail address, School Education and the closest airport of departure.
***Medical Certificate – HSE, AISI, OGUK, NORWAY, etc. The client is requesting 1 year validity, therefore the certificate needs to be issued not earlier than January/February 2012. This request is mandatory for all personnel.
***H2S Certificate – the onboard induction is not accepted, but only the certificate issued  after the training course. (in order to proceed immediately you can apply on web and do the H2S training, most divers have done it on acces.safety.com
****Passport – requires at least 3 consecutive empty pages for the stamps.
